Medicines on prescription from Mendip Country Practice (Coleford Surgery) can be collected from:

 

Mr & Mrs Baker,       10 Park Hayes,    Leigh on Mendip       

Between 9.00am & 5.00pm

 

 

If you do not have to pay for your medicines, you can also take the prescription to 10 Park Hayes, but if you have to pay then you must take it to the Coleford Surgery with the payment.

 

 

When you deliver your prescription, you must say that you want to collect the medicine from Leigh on Mendip.
